Your Health Record Clearance and Management Services for Clinical Pre-Placement

Your choice of career path requires you to meet specific health and safety standards as outlined by Ontario law. The Ontario government and the organizations offering you a clinical placement have mandated specific medical and non-medical requirements that must be met by you, before you can begin your first clinical or work term placement. These measures are there to protect you, your co-workers and those that you will serve in your work placement.

The Requisite Program is the service approved byGeorge Brown College, that will review and validate that you have met these requirements. Our qualified and experienced Registered Nurse will meet with you face-to-face to review your Clinical Pre-placement Health Form and required documents to determine if you have been cleared for your clinical pre-placement.

**Important Notices For All Students**

  • ParaMed office has a strict "Fragrance Free Zone" policy. Kindly do not wear any perfume, lotion or cologne at your scheduled appointment.
  • Due to the high volume of student bookings, there is a strict No Walk-In policy. This is to ensure that students who have booked their appointments online are seen at their scheduled appointment time.
  • Do NOT call to book appointments. Appointments may be scheduled online at www.georgebrownhealth.ca
  • *NOTE* Mask Fit tests can be done on Saturday ONLY if there are at least 10 appointments booked. If you would like a Mask fit test done on Saturday please contact the office.
  • Appointments may be cancelled on-line within 24 hours of their start time. Short notice cancellations must be phoned into ParaMed at 416-348-9185. Failure to provide 24 hours notice will result in a missed appointment fee.
  • Once you have seen the ParaMed Requisite Nurse, please allow 48 hours for the data to be entered into the website.
  • Don't forget to bring your Immunization Record and/or yellow card with you to your appointment along with all other required documents.
  • Mask fit test will be done for you at the same time of your appointment. Please bring your payment and make sure that you do not eat, drink or chew gum two hours before your appointment and male students must be clean shaven. If you are pregnant, you are not allowed to be fit tested and we need a medical note in order for you to get exception.
  • ParaMed Requisite Staff are not available during the lunch hour (12:00-1:00pm)
  • You are required to bring 2 copies of all documents and certificates; your original and a photocopy to your appointment at ParaMed.
  • ParaMed does not accept faxed copies of any documents, with the exception of the seasonal flu shot. If you need to submit new or additional documents, you must schedule a return visit appointment.

ATTENTION STUDENTS: New ParaMed Service Fees June 1, 2017 - May 31, 2018

  • Initial Visit Fee $55.10 (Student pays)
  • Returning Visit due to Deficiency List Form- $24.15 (Student pays)
  • Mask Fit Test Certificate- $41.90 (Student pays)*Note* Only available Monday-Friday, not available on Saturdays
  • Photocopies- $3.00 (Student pays)
  • All prices include HST where applicable

ATTENTION STUDENTS: A $55.10 fine will be collected directly by ParaMed:

  • IF you missed your appointment (without 24 hours notice)
  • The student will now pay the $55.10 fine directly to ParaMed before you can see the Registered Nurse at your scheduled appointment. To avoid any of the above charges, ensure you have completed your specific program health form medical/non-medical requirements before you go to your scheduled appointment at ParaMed.

Additonal Services: Mask Fit Certification Available

  • Students who need Mask Fit Certification can get it done at the same time as their appointments. There is no need to book an extra time slot as this is part of your pre-placement appointment for all students requiring this certification.
  • *NOTE* ParaMed will ONLY provide mask fit testing on Saturdays if there are at 10 appointments booked. Please contact the office for further information.

How to Book An Appointment?

For all first time users: Before you can create an account to book an appointment online with the Registered Nurse, please make sure that you have completed the following requirements:
  • Confirmed your registration into the program or course with a valid Student ID number.
  • Paid your tuition fee either in full or partial and deferral payment.
  • Completed the Clinical Pre-placement Health Form medical immunization records and supporting documents.
